Did you want us to directly use the header and footer you've put in the word document? or do you want us to recreate it? I only ask becuase the header with your logo and name seems a bit pixelated and blurry.

Thanks for your message. Ideally we would like the footer to be designed the same way (as shown in the the document)...and not just simply insert it as a picture.

For the header, I'm trying to get my hands on a high resolution logo but can't seem to find it right away. Please do with the one provided here and I can provide you a high res one later.
