We need a banner to use at conference and different community events. We are a Primary Care practice that specializes in caring for people with diabetes, high blood pressure and people currently on dialysis. Diabetes and High Blood Pressure are the number 1 & 2 reasons that causes people to go on dialysis and we want our banner to convey this message. The majority of people currently on dialysis, have diabetes or high blood pressure are black or hispanic and our clinics are in lower income areas. When people see our banner we want them to think about their health or their family members health and come visit us at the clinic. Such as:
