what information do you want on the brochure? what size are you looking at – a trifold brochure, a booklet type, etc?

Can you give me the dimensions for the brochure size? Will more information need to be on the brochure other than the form? If so, what?

The reply envelope is about 9x4", so as long as the brochure fits in there, it's fine. How much it folds doesn't matter, as long as the form fits and can be torn away and placed in the envelope. - Or not torn away, perhaps the entire brochure would be placed in the envelope once filled out.

The only information that needs to be present is the form one fills out, for which I have provided examples in both French and English.
