Web-Design Kurzbeschreibung

I am commissioning a computer program for use in my office to fill out forms. It will operate over a left and right monitor (i.e. 2 monitors, a left screen and a right screen), with the main user interface on the left monitor (forming boxes in an upside down L shape, with the New File and Existing File boxes forming the top of the upside down L). In the middle of the upside down L, will be a large data entry box (which will include various data fields - I have enclosed 1 sample of the type of data sought). The interior left box is attempting to catch much the same information as the 'Plaintiff's Claim' form attached, but has 3 yes/no questions at the top.

The right monitor will comprise vertical sections running up the left side of the screen. When 1 is selected, it will open a box displaying data from the main user interface.

For the logo (center of the left monitor at the top, please insert CourtDoxx, which should be italicized, slanted to the right, and with Doxx dropped down slightly from Court.

Note that the Statement of Claim form has been uploaded to show what the 2nd monitor is constructing (everything above the words 'Statement of Claim'

Muss haben

• Clean design, easy on the eyes, preferably pastel colours, unless you can come up with something better looking. Designs for the left and right monitor should be submitted together, and they should be consistent with one another.
• Also, the right monitor design must use a Word doc as the background, like in the 'Right Monitor' image. The right screen is to hang off the left side of the Word doc, and will be used to modify small sections of the Word doc 1 at a time.
