In addition to the information you have provided it would be nice to have any attachments of your product, company logo, etc.
With that if you could include things like a preferred color palette and/or examples of designs you like, how you would like your company to be perceived,etc.
That information would be great to have in order to give designers a good starting point and an idea of what you are looking for.
