We need a 3.75" wide by 8.25" long patient referral card that a physician will fill out to refer their patient to Sound Cancer Care. The front of the card will have our logo on top, followed by contact information, a map that shows the office location and then there will be descriptive text at the bottom. On the back side, there will be a headshot photo with a brief bio of Dr. Kevin Sanders. The address for the office is: 2920 S. Meridian, Puyallup, WA 98373.
