App-Design Kurzbeschreibung

We need a sharp professional design for a phone application. We specialize in Health and Fitness Software. This app will be a companion for www.ExerciseProLive.com.

We will be doing all backend and javascript work here and we need someone to create the HTML and CSS for the application which will be developed in Cordova.

For the contest we are submitting three screens. Once we award a winner we will give the winner the first oportunity to bid on the remaining 10 screens.

For the first step of the contest please do the page 1 and page 2 screen design, feel free to submit multiple designs. If we like your designs we will contact you and ask you to design the third page. Once that is done we would like to see the HTML and CSS for the three pages.

Page 1: Simple Home Page
Page 2: Client Search - The search is at the top and the results are below that. The Active | Inactive section is a selection (see https://thestizmedia.com/radio-buttons-as-toggle-buttons-with-css/ for an example). Client A has their record expanded (that is what the triangle is). 136 stars should be 136 with Font-Awesome star after the number. To see what the graphs should look like please see the attached png. Then a list is below the stars and graphs.
Page 3/4: Step 1 of 3 - When the magnifying glass is selected a flyout with the search criteria comes out (page 3). Criteria is selected and the results are shown (page 4).

The default width should be 320 but the application should look good on all phones and tablets.
