Word-Vorlage-Design Kurzbeschreibung

I am creating a Nursing Assessment Form (which is used to be an intake form for all new patients) for my new home care company, Community Choice Home Care.

It is 15 pages and pretty comprehensive as this is a very regulated field. I have all the necessary content and information attached in the word document below. Now, I am seeking a designer to make it look professional, cohesive and easy to read/ follow.
In order for your Word Document Design to be selected, you must:

1. As mentioned previously, make it easy to read/follow. There should be enough spacing, room to write out all necessary notes, fill out check marks...etc...
2. It should look professional.
3. It should be attractive and eye pleasing. Attached you will find our logo-kindly incorporate it's style into the project as well. The logo should be placed on top of each page.
4. Lastly, we need a Word Document version AND PDF Fillable Option. The fillable PDF should Auto Fill for the Name and Start of Care (SOC) Date.

I have attached a sample form below as well. This is how I envision the finished project to look. Of course, I am open to other suggestions as look as it meets the aforementioned requirements.
