Grafik-Design Kurzbeschreibung

We are a group of 7 care home and we are building our new website.
On the "where we are" page, we want to have an image with the "map" of the different locations of our care homes. The client will be able to hover over each location icon with the mouse and see what a specific location is about.
The task is to design a conceptual map that will allow the client/user to immediately understand where approximately each location is (hence to hover over the one that suits them the most) . We tried with a regular map but the result was poor given the actual position of the icons on the map hose are the actual locations we want an icon for.
The closest idea to what we have in mind is the map of the London Underground. It is schematic, conceptual, gives an accurate representation of where each stop is in respect to the others, but is is not a geographical map. In the required design, however, some geographical elements (main cities, motorways etc...) need to be present to allow the user to understand where each home actually is. We are not ruling out the geographical maps, it's only that we weren't able to make it work.
