If you want to convert the design into email compatible HTML file then you need to add additional budget for the job. The Here HTML/CSS is comes under coding job.

Another issue is: I don't think Gmail allows for input fields in emails so the best approach for this would be an email blast to book an appointment then clicking on it redirects to the landing page with the input fields that you want.
