App-Design Kurzbeschreibung

I need small businesses to enter a self-assessment. The self assessment link should be found on my WP website. The app needs to come in company colors (pre defined) needs to contain 6 sections of 3 to 6 questions. the average result per section should be visible in a (radial bar chart) graph after entering the last question.
Each section should have informative picture upload possible and information button whit text if you hoover over it. The page should have a title descriptive text. The number of of answers to give per question is 1 . Answer is either, 1,2,3,4 or 5 (likert scale) each page should have a back and forth button. There should be as well a non mandatory contact form. The results of the questions can be collected in excel or google sheets for our own use. The average result graph should become visual after answering all the questions. Text that comes with it should be conditional based on the outcome of the answers. Other option is that these results are sent by email to the submitter in pdf.
