Flyer-Design Kurzbeschreibung

We need a tri fold brochures. One that explains our home based (nurse home visits) palliative care program to patients and their families. It should explain what palliative care is, who is on the team, and how we support patient and their families. We also need one geared to Oncology groups that focus on cancer care and another one for regular physicians groups letting them know that we can serve their patients who have an advanced disease such as cancer, congestive heart failure ,Parkinson’s and Alzheimer’s disease. This material should market our home based palliative care program for different audiences.
