Web-Design Kurzbeschreibung

I need a website where prospective and established patients can obtain information and then direct them to making appointments. My practice is a highly professional surgical practice specialising in surgery to the foot and ankle. My title is Mr Peter Manuel, Podiatric Foot & Ankle Surgeon. My qualifications are: BSc MSc FACPS
The practice website will require information on: Locations, phone numbers to make appointments, general background on podiatric surgery,
I would like to include information on: my hospital appointments, surgical fellowships held, training programs undertaken, association memberships, publications written, links to useful websites
A page dedicated to common foot conditions would be useful and also a page with referral information including an online registration section that will be sent direct to the receptionist.

I will provide files on logo, self photos and my personal training history etc.
Also if there is going to be a background theme I personally am associated with surfing and beach activities, it would be great if this was able to be reflected in the design.
