I'm very much intrested in this project. Just wanted to clarify, whether you want the text flow as you have given in the document? And what about the content given in the pdf format..
I can design a few pages for sample and once approved I can give you a complete design.

Hi Jai:
Text flow should be as given and the PDF is a previously designed Appendix. I like your idea of designing a few pages for sample - if we like your work, we have images that can be used in the document.
