Need a business card for a surgical practice called Modern UroGynecology. We have recently selected a logo (attached). The business card will need to have two formats:
1) One with the logo/physician name/ contact info on the front and location information on the back.
2) One in "appointment reminder card" style with the logo/ contact info/ date/ time on the front and the location information on the back.
The front side should include the following information:

Thanks everyone for the hard work -- it is appreciated. I think that I would like to see some simpler designs. I will upload another file with a design that can be modified -- it simply has a blue design faded to the center. Perhaps a variation of that might work for the front of the card with the following format:
