Product label for herbal salves, lotions, soaps and tinctures

We need a very simple, clean and organic looking label that can be used for an ointment jar, a lotion bottle and a dropper bottle.
We have an existing logo (see attached). We like the colors white, olive green, gold and black. It needs to include product name as dominant and logo. It also needs to include an ingredient list, and usage statement. Include weight of product, distributed by, made in the USA, and FDA statement.
