If 3d template is made in 3ds max, does it work you?

unfortunately - I am not familiar with 3DS Max. I know Illustrator and Photoshop.

If the 3DS can be exported to 2D, with a 3D like feel, that'd work. Also, if 3DS template is easily updated and I can engage you or a designer for mock ups from the template with different color and logos on a small fee basis, that might work as well.
